Brokers please submit the following documentation for all loans to be underwritten:

• Submission form completed and signed
• Loan Application (1003) signed and dated by the Borrower(s) and Loan Officer
• Loan Transmittal Summary (1008)
• Initial GFE (2010 version)
• GFE Settlement Service Provider List
• Truth-In-Lending Disclosure
• Borrower Signed Credit Card Authorization Form (after 2-15-10 if applicable)
• Title Bill
• Purchase Agreement/Sales Contract (Purchases only)
• Credit Report
• Income Docs (Salaried: 2 yrs W-2, 1 month paystubs; Self Employed: personal and business tax returns for 2 yrs (all schedules) and evidence of extension if applicable
• IRS 4506-T form, completed and signed by the borrower(s)
• Credit Explanations for all derogatory items
• Bank Statements
• Valid Driver’s License
• Social Security Card
• DU Findings
